Donation Request Information
Although I would love to fulfill every request for a donation, due to the volume of requests received, I am simply not able to do so as a small business. If I am able to donate to your event I will contact you as soon as a decision has been made. If I am unable to participate for your current event, please feel free to submit a request for future events.
Please include as much information about the event as possible, information such as: date of event, place event will take place, purpose of event, coordinator of event, tax ID number, address, phone number & email address for correspondence. Donation requests are reviewed at the end of the month so please allow plenty of time prior to your event for ample consideration.

A debit/credit card on file and 50% retainer is REQUIRED to schedule an appointment; retainer will later be applied to the final treatment payment.
Retainer for treatment is NON-NEGOTIABLE and NON-REFUNDABLE.
To RESCHEDULE/CANCEL an appointment, please provide 72 hours notice. *** Failure to provide 72 hours notice will result in forfeiture of retainer and a new retainer will be required for any future scheduling of appointments.***
***ONE RESCHEDULING of appointment will be given and the transfer of your retainer to that rescheduled appointment. RESCHEDULING your rescheduled appointment will RESULT in another 50% retainer being REQUIRED.***
There is a 15 minute grace period if running late; if you arrive AFTER the grace period, your retainer will be forfeited and a new retainer is required for future appointments.
Remaining balance of treatment(s) can be paid in CASH or your CREDIT CARD on file.
Please, NO extra guests will be allowed in waiting area unless previously discussed with me.
NO CALL/ NO SHOW APPOINTMENTS WILL BE CHARGED 45% OF TOTAL SERVICE COST AND NO LONGER ALLOWED TO BOOK WITH ME.
